In this report, we described a newly devised quantitative method of red blood cell (RBC) surfaced-bound IgG and RBC IgG autoantibody in the serum of patients with autoimmune hemolytic anemia (AIHA) by fluorescence enzyme immunoassay (fluorescence-EIA). While the enzyme linked immunosorbent assay (ELISA) technique is most widely applied to quantitation of various antibodies, fluorescence-EIA is a more useful method in which fluorescence analysis integrated into ELISA in order to increase sensitivity. This method was applied to RBC from normal subjects (7cases), and to patients with AIHA who showed positive and negative in the direct Coombs' test (12cases). Relative fluorescence units (RFU) increased in proportion to the positivity of the Coombs' test, and both data correlated with a correlation coefficient of 0.698 (P<0.01). Red cell IgG autoantibody in the serum was measured as an IgG autoantibody towards O type RBC from the normal subjects. The antibody of patients with AIHA (14 cases) demonstrated in about half of the patients in spite of negativity of the indirect Coobms' test. In one case of AIHA probably induced by Rifampicin or Ethambutol HCI, red cell IgG autoantibody in serum was measured in the progress of disease. The indirect Coombs' test were negative throughout the obsevation, but the level of autoantibody in the serum was higher than for normal subjects, and it lowered to a normal value with the recovery from illness. |
