Virchow転移が消失した症例に対し原発巣を切除しているが ,この切除症例について手術の妥当性を検討した .【対象および方法】 2005～2012年に , Virchow転移がある胃癌で化学療法後原発巣を切除した 4例を対象として ,臨床病理学的に検討した .【結果】化学療法は S-1を中心に 6-11カ月行い ,原発巣を切除し傍大動脈周囲リンパ節のサンプリングを含めたリンパ節郭清を行った .4例中 2例が深達度は mと smであり ,リンパ節転移はなく ,術後 4年 9カ月と 8カ月無再発生存中である .2例は ssと seであり ,リンパ節転移があり術後 1年と術後 8カ月で原病死した .【結語】化学療法後 Virchow転移が消失した胃癌の場合 ,長期生存が期待しうる .リンパ節は癌が消失しても原発巣には癌が遺残することが多いので ,原発巣は切除したほうが良い .病理学的にリンパ節転移がない場合 ,長期生存の可能性があるが今後の症例の集積が必要である
