Article","subitem_description_type":"Other"}]},"item_6_description_4":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"【目　的】　脳梗塞急性期に施行した頚部内頚動脈外科的血行再建術の特徴，治療成績を評価し，治療方針の妥当性について検証する．\n【対　象】　2016年10月～2019年9月の間，頚動脈内膜剥離術（carotid endarterectomy：CEA），頚動脈ステント留置術（carotid artery stenting：CAS）を施行した頚動脈狭窄症連続30例を後方視的に調査した．\n【方　法】　最終脳虚血イベントより14日以内を急性期と定義，非急性期群と比較検討した．2群間での患者背景，術前画像所見，病理所見（CEAのみ），合併症，転帰につき調査した．\n【結　果】　急性期群は9例（CEA4例，CAS5例），非急性期群は21例（CEA10例，CAS11例）であった．術前画像所見に両群で有意差はなかった．永続的合併症，治療30日以内の同側脳卒中再発，虚血性心疾患発症は両群で認めず，治療30日後のmodified Rankin Scale（mRS）0─2の割合は急性期群で3例（33％），非急性期群で16例（76％）であった（P<0.05）．全30症例でCEA施行群ではCAS施行群よりも有意にプラーク体積が大きく（CEA：1,090 mm2 CAS：612 mm2
